What we do For adults Memory support Hertswise Dementia Service About the service Hertswise is a service for people living with dementia (with or without a diagnosis), low level memory loss or mild cognitive impairment and their family, friends and carers. It is provided by a partnership of 9 community and voluntary organisations. Hertswise aims to ensure that people of all ages, living in Hertfordshire can access information and advice, activities and support. We offer the following services to support service users to make positive changes to their physical and emotional wellbeing. Community Network Plus Groups 2 hour, weekly run groups in 10 localities/districts across Hertfordshire – facilitated by the Locality Worker. In these groups we use the 5 Ways to Wellbeing model to promote positive health and wellbeing. Activities are based on what clients want to do – making the session person-centred. Community Network Groups 10 week, 1 hour groups run in a variety of different settings/locations across Hertfordshire from sheltered housing, care homes, community centres, churches etc – facilitated by the Locality Worker. In these groups our aim is to promote socialisation, positive lifestyle changes, confidence building with the hope that the service users will continue to meet a group once the 10 weeks are over. We can give the client an arm-length support to help them to continue to meet on a regular basis. We also provide the group with resources to further empower them to continue as a group. Locality workers will plan activities based on the service users likes and interests with the aim of service users becoming confident enough to join Community Network Plus groups. Carers Support We have a carer support worker as part of the project who will move around all the groups that are facilitated as part of Hertswise and speak with the carers and try and fulfil their unmet needs – ensuring that they are signposted for further support if the carers need this. Community Information Network (CIN) Advisors offer support and advice on benefits, Lasting Powers of Attorney, respite care and may other things and signpost clients/carers on to the right support services. The CIN workers can visit clients and carers in their homes to support them with completing benefit forms etc. How to contact us? It is an open referral route (via self/professional/community).
