Our Bounce Back service provides one-to-one outreach support to facilitate a smooth, timely and supportive discharge from hospital back in to the community. We provide advice, information and holistic outreach support to people who are experiencing mental ill health or need help with their mental wellbeing. Bounce Back can provide 10 sessions of support, but there is flexibility for clients with multiple needs who may require longer term support.
