Physical Health Checks Service
About the service
Our Physical Health Checks service supports people living with severe mental illness (SMI) to improve their physical health and wellbeing. We understand that accessing healthcare can sometimes feel challenging, so our Community Outreach Workers provide practical, personalised support to help people receive the care they need.
We work closely with GP practices, the Community Mental Health Team, healthcare professionals and community services to reduce health inequalities and support long-term wellbeing.
How can we help?
The sessions will focus on your needs, to support you with the areas that are most important to you.
We can provide:
- Physical health checks in healthcare and community settings, and through home visits
- Practical support to overcome barriers to healthcare access
- Support to access and attend follow up healthcare appointments and community-based support
- Liaison with GP’s, healthcare professionals, family members and support networks
- Guidance on improving physical health and wellbeing
- Help understanding health check results and next steps
- Signposting and referrals to relevant health, wellbeing and community services
- Opportunities to connect with community activities and services that build confidence and wellbeing

The service is available to adults living with severe mental illness who are registered with participating GP practices across
- Watford
- Borehamwood
- Hemel Hempstead.
